This case was presented at Santiago, Chile in November 2008, where Dr. Jin Kim was an invited speaker for Sociedad de Implantologia Oral de ChileSociedad de Protesis y Rehabilitacion Oral de Chile, at their annual meeting. 

 

The case involves complex treatment sequancing involving clinical periodontics, orthodontics, and restorative disciplines in order to achieve an ideal anterior outcome. The restorative partner was Dr.Margareta M.Gavrila of Chino, CA.

Block onlay graft (J-Block, Tutogen) was secured with Biohorizons GBR screws.

A 1 x 4 Alloderm (acellular dermal matrix) used in conjunction with platelet-rich plasma (PRP).

The two bridge abutment teeth were reprepped to bring finish lines more coronally. This facilitated root coverage with soft tissue graft, by shortenening the clinical height of the teeth.

A Biohorizons Internal implant (3.5mm x 12mm) was surgically placed.

Sub-epithelial connective tissue graft was performed.

After 4 months of osseointegration, proisionalization and 'tissue grooming' is initiated with screw retained PEEK (polyetheretherketone) abutment.

Crown lengthening og the right anterior teeth to achieve symmetry.

Zirconia abutent (Atlantis) and provisionals.

Orthodontic therapy was needed to mesialize the left central incisor. Approximately 1mm of movement was acheved over an 8 week period of fixed appliance therapy.
